Information

YOUR FIRST VISIT
What to expect:
You will be greeted and
welcomed to our clinic. The first appointment will be
approximately fifty minutes long. You will meet your therapist,
described your injury or situation, and learn about your plan of
treatment. Your therapist will begin your first treatment at that
time.
What to bring:
Please bring (or fax to 207-883-6199) your prescription for treatment
from your doctor, a list of the medications you are taking, and
insurance card(s) or your worker's compensation/auto insurance information.
If your insurance policy is referral-based, it is your responsibility to
obtain the referral from you primary care physician and either bring it
with you or have it faxed to our office.
What to wear:
Wear
comfortable clothes that will allow for flexibility. Sweats, shorts, and t-shirts work especially well, and
you are welcome to change at our clinic.
Frequency of visits:
After your initial visit,
we will be able to determine how often you will need to attend physical
therapy. We will consider your physical therapy needs as well as
your schedule and time constraints.
If you are scheduled
for an initial appointment, you can get a head start and save time by
downloading and completing the following forms:

Cancellations:
We ask, as a courtesy to
our physical therapists, that when you do need to cancel an appointment,
you provide us with a twenty-four hour notice, if possible. We
understand that certain circumstances are unavoidable. If you are
late for your appointment, you may be asked to reschedule for another
day, or if you are not present for a scheduled appointment, you may be
charged a fee.
